Fill out the form with necessary information. This information will be used to confirm your appointment and send you the necessary details via email, including a video link for Virtual Appointments.

Step 3: Get Ready!

You will receive an email confirmation with your Virtual Appointment link and confirmation. We recommend being on a reliable internet connection.
